One Medical Testing On Campus
We’re pleased to announce that beginning Wednesday, November 4, One Medical will offer on-campus testing for students and faculty/staff.
You can still book your COVID-19 PCR test at the location of your choice; however, as we have now moved to weekly testing for any individual planning to spend time on our campus, we hope this convenience comes as welcome news. The details below outline how testing at Menlo will work during the hybrid weeks through November. We will send updated information after this initial hybrid session has concluded.

Testing Consent Form
Before your child can be tested on campus, you will need to sign and return this Legal Guardian Consent Form. Your child can bring the signed form to their testing appointment, or you may submit it online through the Messages in your One Medical app.
Students planning to be on campus should test every seven days.
Testing will take place in the lobby of the Spieker Center for the Arts.
Note new arrival and parking info: Coming from off-campus just for One Medical PCR testing? Please enter through the Alejandra Ave entrance at Wunderlich Field and park in the lot adjacent to the field. Signs will direct you to the testing center located in the lobby of the Spieker Center for Performing Arts. Do not enter other parts of the campus before or after your test. Please remember that parents are not permitted on campus at this time. You can also schedule a test at another One Medical location that is convenient to you.
Middle School Students
For your convenience, Middle School students will be pre-booked with appointments on either Wednesdays or Thursdays. One Medical will notify you by email of these appointments, which will also show as upcoming appointments in the app. If the pre-scheduled time doesn’t work, you can reschedule in the app or by phone.
Please wait until 3:00 pm Friday to review or change your confirmed appointment time. If you don’t see any confirmed appointment time in the app by then, please reach out to firstname.lastname@example.org.
Upper School Students
Students should independently schedule their testing appointments through the One Medical app, where Menlo should now be listed as a testing location. Campus appointments will be available on Wednesdays and Thursdays, 8:30 am-4:30 pm, and Upper School students can choose a testing time that fits their schedules. Please try your best to stick to the same day each week, appointment availability and class rotation permitting.
Students should book an appointment the week before their scheduled on-campus learning as well as the week they are on campus. Students enrolled in after-school enrichment will also need to schedule a test for each week they are on campus, whether participating in on-campus learning or not. Upper School students will be responsible for going to their appointment on time.
If you don’t see Menlo’s on-campus testing site in the OneMedical app, double-check that you signed up using Menlo’s sponsored code: MLOXMED. Signing up using this code identifies you as a part of the Menlo community and allows you to see our private testing site.
One Medical Badge and Submitting PCR Test Results
- In addition to testing every seven days, everyone who comes to campus should continue to complete the One Medical daily symptoms tracker to get their badge. Only green badges will be accepted on campus.
- When you get your PCR test results, please continue to upload them to the School nurse using this form.
- You can cancel or reschedule appointments in the One Medical app or by phone as needed.
